If you do not accept this agreement in its entirety then you may not visit, access or use the alcure.me website or Alcure digital health platform.
Alcure may modify the terms of this agreement at any time and will post modifications on this page of the alcure.me website. Modifications are effective once posted and correspond with the stated date of the modification or "Latest version" of this agreement. It is your responsibility to carefully review this agreement each time you visit the alcure.me website or use the Alcure digital health platform
You affirm that you are at least 21 years old and a resident of one of the states we service as published on the alcure.me website to use the Alcure digital health platform. Because of the nature of the telehealth services we offer, Alcure is not able to provide service to anyone under the age of 21 and upon notification or discovery that any user is under the age of 21, Alcure must immediately discontinue service. Alcure also requires all users of its digital health platform to provide honest and accurate information at all times and failure to do so will result in immediate discontinuation of service.
3. Service offered and consent to telehealth
Alcure is a telehealth platform for physicians and credentialed counselors and coaches (collectively "medical providers") to work with you in a more accessible and efficient way using pharmaceutical extinction (also called pharmacological extinction or the Sinclair Method) to treat Alcohol Use Disorder. All medical providers are independent contractors and are not employees of Alcure. The role of the alcure.me website and digital health platform are to facilitate the medical providers' interaction with you. All treatment decisions are handled exclusively by medical providers.
Medical provider services are provided exclusively through telehealth, which refers to online communication between medical providers and patients, including video chat, email, texting, and telephone communication. Telehealth also refers to the electronic prescribing of medication to patients and maintaining patient medical records privately and electronically.
Medical providers using the Alcure telehealth platform do not provide "in-person" medical services to patients but, depending upon the needs of a patient, may refer a patient to another health care provider for in-person medical services. The Alcure telehealth platform is an acceptable mode of delivering the health care services described on the website, which consists of the prescribing an opioid-blocking medication using targeted dosing of the medication combined with professional counseling to reduce alcohol desire or cravings and bring alcohol consumption under control.
Treatment on the Alcure telehealth platform may also include group video chat sessions with your counselor or coach which involve anonymous participation or inclusion of your first name or first initial, or an alias name or initial. You consent to group therapy or coaching via group video chat sessions using this form of identification in group video chat sessions with your counselor. For group sessions, you also agree to maintain the privacy of other participants by not revealing to outside persons their names, aliases, or other information that may permit an outside person to identify them.
Some clients will elect for coaching support. Coaches are also independent contractors and are not employees of Alcure. Coaching helps clients in the process of autonomous treatment. Coaches are well-versed in Alcohol Use Disorder and helpful strategies to assist with the treatment provided on the Alcure digital health platform, but coaches are not credentialed counselors and cannot diagnose or treat mental health issues.
Communications with all your medical providers on the Alcure digital health platform and Alcure support are private and protected under doctor-patient and counselor-patient legal privileges to enable you to speak freely to your medical providers about your health conditions. To the extent individual privileges extend to each provider, you agree that all providers are collaborating to optimize your treatment and will share your medical information between them, which is not uncommon, and waive applicable privileges as necessary to carry out this collaboration purpose. The waiver extends to Alcure support which may need to access confidential information in the same way as a medical provider but for administration purposes only. Any disclosure of privileged information to other parties will only be done in accordance with applicable laws. Communications with coaches are subject to a general right of privacy but may not be legally protected from disclosure in the same manner as doctor-patient and counselor-patient communications.
All charges collected from you are allocated for services provided to you in using the Alcure digital health platform and are not allocated to any product or goods to include, but not limited to, the medication delivered to you or our digital product consisting of our tutorial video modules.
The alcure.me website nor any advertising associated with it or the alcure digital health platform, including written content, imaging, and any video imaging ("content") are for informational purposes only. Content is not a substitute for medical advice, diagnosis, prognosis, or treatment. Any questions you may have regarding any health-related condition should be immediately addressed with a qualified health care provider, and you should not disregard medical advice or delay in seeking medical advice based on any content published by Alcure. If you believe you are someone you are with is facing an urgent medical situation or emergency, call 911 immediately. Alcure does not provide urgent medical advice or services through the alcure.me website or its digital health platform.
Alcure is not a pharmacy. Alcure contracts with a licensed, U.S. pharmacy that fulfills medication prescriptions for Alcure clients after a prescription is issued by an Alcure affiliate physician licensed in the state where the client resides, including express delivery of the medication to the client. Though Alcure strives to deliver medication in three business days or less, it cannot guarantee a delivery date and delivery can be affected by a variety of factors beyond its control including, but not limited to, incorrect delivery address information, payment rejection, or unforeseen events which may cause medication delivery to be delayed or disrupted.
Alcure is also not a laboratory. From time to time its contracted health care providers may require you to obtain laboratory tests to continue with service. Such tests are provided through a contracted laboratory and priced separately. Alcure will advise you in advance of any additional medical services, including lab work and prescriptions for medication designed to mitigate potential side effects of the primary medication it prescribes to treat Alcohol Use Disorder, including pricing.
The Alcure digital health platform treats Alcohol Use Disorder using pharmaceutical extinction (also called pharmacological extinction or the Sinclair Method). It is not an all-purpose digital health platform and does not treat other physical ailments except those related directly to pharmaceutical extinction which includes, but are not limited to, primarily potential side-effects of the medication we prescribe.
The telehealth services should not be considered your primary source of medical care and are not intended to be a substitute for care with a primary care physician. As such, you should advise your primary care physician of your care with Alcure and all medication prescribed by Alcure, and also advise your health care provider on the Alcure digital health platform of all medications are taking and all medical conditions you have to optimize care and reduce the risk of contraindicated drug interactions or treatments. In other words, be transparent with all of your health care providers, including those on the Alcure digital health platform regarding all medications, whether prescribed or over-the-counter, legal or illegal and all medical conditions you have for your safety. You may elect to have your Alcure health care provider communicate directly with any other health care provider by contacting email@example.com.
Though Alcure provides a robust technology supporting its digital health platform, it cannot guarantee against risk in loss of electronic data, delay in electronic transmissions and communications, quality in electronic transmissions and communications, third-party "hacking" or data loss.
Alcure requires all clients to answer a health questionnaire that includes more detailed terms regarding (1) consent to telehealth treatment, (2) privacy, and (3) informed consent for treatment after clicking on the "Start" button on any of the pages of the alcure.me website and all terms set forth in each of these forms are incorporated by reference herein.
